New Patient Registration

To register with the practice please ask at reception for a registration pack. Complete the forms and bring them back to the surgery. Our practices cover an area within Worle and Weston-super-Mare and we have a boundary outside which we cannot accept patients.

New patients will also be asked to complete a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can also give consent for someone else to collect results or other confidential information on your behalf and if you would like to nominate a specific person, please ask at reception for a form.

We will also ask you if you would like to register for online appointments, prescriptions and medical record access and request you to complete an application form if you would like to use this service.

We offer a text messaging service for appointment reminders and other healthcare information. From the 1st of May, you will need to give your explicit consent and will be asked to complete a consent form.

Information on how your medical record information is shared with other healthcare providers will also be provided within the registration pack and this should be read and completed where appropriate.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Improved Access

We are working together with other local practices to offer patients access to more appointments in the early mornings, evenings and at weekends. To find out more about these services, please contact the practice.
