Summary Care Record

Your GP surgery needs permission to share important information from your GP record. This is called a Summary Care Record (SCR). Your SCR can only be shared with health and care staff across England who are providing you with direct care. It gives them access to vital information from your GP record.

Please note, the decision you make about your Summary care Record (SCR) is separate to the National Data Opt-Out. The SCR is information shared from your record for the purpose of providing direct care and treatment to yourself as a patient only. The National Data Opt-Out is the choice you can make to stop your confidential patient information from being used for research and planning of NHS services.

National Data Opt-Out

The ‘National Data Opt-out’ was introduced on the 25th May 2018, providing a facility for individuals to opt-out from the use of their data for research or planning purposes. This is provided in line with the recommendations of the National Data Guardian in her Review of Data Security, Consent and Opt-Outs. The NHS has launched the national data opt-out as part of the ‘Your Data Matters to the NHS’ campaign.  This campaign informs the public that the strict rules about how health and care data can and cannot be used have been strengthened. The NHS is committed to keeping patient information safe and always being clear about how it is used. 

If you don’t want your information to be used by the NHS for research and development, you can register your decision to opt-out online Choose if data from your health records is shared for research and planning - NHS (www.nhs.uk), through the NHS app under the ‘Your Health Choices’ tab, or via post:

If you are happy for your confidential patient information to be used by the NHS for Research and Planning, you do not need to take any action.

If you decided to not have your confidential health data used in the Nation Data Opt-Out when registered at your previous practice, you do not need to repeat this process when registering with Cedar Surgery, unless you have changed your decision.
